I hereby release and agree to hold TES harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses, and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the Summer Camp, or that may otherwise arise in any way in connection with any services received from TES. I understand that this release discharges TES from any liability or claim that I, my heirs, or any personal representatives may have against the salon with respect to any bodily injury, illness, death, or medical treatment, or property damage that may arise from, or in connection to, any services received from TES. This liability waiver and release extends to Tayba Elementary School together with all the owners, partners, and employees.

I hereby grant TES' All-Star Summer Camp the right to reproduce, use, exhibit, display, broadcast, distribute and create derivative works of the photographed images of me and my children, taken during Camp, for use in connection with the activities of Tayba Elementary School or for promoting, publicizing or explaining TES or its activities. This grant includes, without limitation, the right to publish such images on TES' websites, in our reports and publications and PR/promotional materials, such as marketing admissions publications, advertisements, fund-raising materials, and any other TES related publication. These images may appear in any of the wide variety of formats and media now available to TES and that may be available in the future, including but not limited to print, broadcast, video, and electronic/online media.

I confirm that my child will respect TES' All-Star Summer Camp, the space, and its employees, and that failure to do so will result in my removal from the program. I accept that neither TES' All-Star Summer Camp, nor it's facilitators, volunteers, staff, directors, contractors, and/or affiliated members are labile for any injury or damages to my child or their property resulting from participating in activities or staying on site.
